I ground the jaws a bit and used a puller similar to DMadigans. You can also use a worm type hose clamp around the puller jaws to help prevent the jaws from slipping off the flats on the pinion. Put some tension on the jaws, a little heat from a pencil torch and a tap will get it moving. It is all just part of the fun of working on these old bikes...a whole lot more fun than a modern car!
